Intake Coordinator

Prevention Programs provide counseling and case management services to families to help them develop strategies to thrive and succeed together as a family and not have their children removed and become part of the child welfare system. The Intake Coordinator will be responsible for taking, monitoring and tracking referrals for all Bronx prevention programs to ensure that the programs are fully utilized. The Intake Coordinator will actively outreach and do presentations to ACS and community resources to generate referrals.
Reports to: Senior Program Director
Location: 2471 Morris Ave. Bronx, NY 10468
Hours: 35 hours / Non - Exempt
Salary: $45,000 - $50,000
Major Duties:
- Works very closely with the 3 Bronx Prevention Program Directors to ensure referrals meet all program requirements and utilization needs.
- Screens all telephone and walk-in referrals and assigns to appropriate program.
- Maintains a shared online referral log system for each program.
- Redirects inappropriate referrals to other resources.
- Follows-up with program on referral status.
- Ensures all referrals have a disposition within 10 days.
- Enters and tracks all information into the PROMIS database system.
- Successfully registers Advocate cases into PROMIS database system.
- Troubleshoots challenges with Advocate cases.
- Participates in ACS Early Engagement Conferences, when possible, to assess appropriateness for prevention services.
- Initiates, builds and maintains appropriate relationships with ACS liaisons and community contacts.
- Participate in community, agency and staff events when necessary
- Any other duties, where appropriate.
Qualifications:
- BA required, MSW or related master’s degree preferred.
- 2 years’ experience in child welfare setting required, prevention experience a plus.
- Spanish speaking preferred.
- Knowledge of PROMIS and Connections.
- Experience with SSO-SMP systems a plus.
- Must have a commitment to work from a strength based and/or youth development perspective.
- Basic computer skills; Microsoft, Excel and Outlook.
