Reports to: Vice President of Clinical Practice Implementation

Location: Brooklyn and Bronx

Hours: 20 Hrs/Wk (3 days a week)

Salary: UNPAID

Major Duties:

• Co-Create Treatment Plans: Collaborate with foster youth, biological parents, foster families, and interdisciplinary teams to develop individualized treatment plans that address behavioral, emotional, and developmental needs.
• Participate in Planning Milestones: Join Service Plan Reviews, case conferences, and team meetings to ensure treatment goals are aligned with Article 29-I standards and Medicaid requirements.
• Engage in Field Work: Conduct home visits and community-based sessions to support youth and families directly in their environments.
• Facilitate Family Connections: Provide psychoeducation, transitional counseling, and support services to foster and biological parents to promote stability and reunification.
• Document with Purpose: Maintain clear, strengths-based documentation that reflects progress, challenges, and next steps in care.
• Collaborate Across Systems: Work alongside case planners, sociotherapists, and health services staff to ensure holistic, trauma-informed care.

Qualifications:

• Enrolled in a Master of Social Work (MSW) program.
• Passionate about child welfare, family systems, and trauma-informed practice.
• A strong communicator who values empathy, cultural humility, and collaboration.
• Organized and committed to ethical, confidential practice in line with HIPAA and NYSED guidelines.
