Family Worker

Location: 2471 Morris Ave. Bronx, NY 10468

Hours: 35 hours Non-Exempt (Evenings are Required

Job Summary: The Family Counseling for School Success Program provides counseling and case management services to families connected with PS/MS15, PS291, PS 246 or live within the community. The Family Worker will provide supportive case management services to assigned families utilizing strength-based engagement strategies. The Family Worker will work closely with the Case Worker/Social Worker to ensure effective and efficient coordination and collaboration of services.

Case Practice Responsibilities:

  • Conducts a concrete needs assessment for all assigned families.
  • Connect families to resources to address identified needs.
  • Assist participants to successfully connect to resources, such as completing application and referral forms, helping with transportation and/or accompany to appointments; etc.
  • Follow-up with providers on status of referrals and ongoing services.
  • Create and establish connections to varied community resources.
  • Conduct home visits.

Administrative Responsibilities:

  • Actively participate in weekly supervision to develop counseling and case management skills.
  • Meet regularly with assigned Case Worker/Social Worker of each family assigned to update on status of case activity.
  • Attend and participate in unit and staff meetings.
  • Complete all required ACS and GSS trainings.
  • Prepare appropriate statistical reports (PROMIS).
  • Complete progress notes within 5 days of event.
  • Share resources with colleagues.
  • Attend community resource fairs to share and connect with community providers.
  • Perform other duties as assigned.

Qualifications:

  • High-school diploma/GED Required. College Graduates are preferred
  • In lieu of candidates without a degree must have extensive experience with accessing community resources. Experience with child welfare and/or school systems a plus.
  • Bilingual (English/Spanish) required.
  • Must be able to work two evenings per week until 7:00 p.m.
  • Excellent organizational, written, and communication skills.
  • Must be proficient in Microsoft Office
  • Must have a commitment to work from a strength-based, trauma-informed, anti-racist, and youth family development perspective.
